***If you list vacation or unavailable dates on your questioinnaire, we automatically excuse you for those dates and you will not receive a Notice to Appear during that time frame.*** Once you do receive a Notice to Appear in the mail (which will be mailed approximately 2 weeks before your jury service), you will receive specific instructions regarding the date to appear and when to call into the automated phone line: (319) 286-2302 - Cedar Rapids and (712) 233-8912 - Sioux City.
